Use this service to get your additional first sick (fit) note for work following an absence of more than 7 days.
You can use this service if you:
- are registered at the surgery
- have already had a sick (fit) note for this absence
If you require a sick (fit) note related to a hospital procedure such as an operation, contact the doctor responsible for your care. Hospital doctors are required to provide sick notes in this instance.
Sick notes can be backdated so any time off will be accounted for.

We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
