Accessing your health record

From 1st November 2023, you can see information held by the practice via the NHS App, the app is easy to download – you can see consultations, results, prescription requests (including when the prescription has been issued) and all aspects of your record.

To see any of your records or have a copy of your records prior to 1st November 2023, please complete the access to medical records form and return this via the practice email gmicb-mh.chorltonfamilypractice@nhs.net or print this out and bring it to reception

In accordance with the UK GDPR, patients are entitled to receive a response within the maximum given time frame of one calendar month from the date of submission of the SAR.

To ensure full compliance regarding requests, this organisation will adhere to the guidance provided in the UK GDPR. In the case of complex or multiple requests, the data controller may extend the response time by a period of two months. In such instances, the applicant will be informed in the first month and the reasons for the extension given.

You can access your health record and nominate someone you trust to access it too. You will need to contact the practice.

Access to other medical records

For any other medical records, you need to ask for them at the NHS service you went to. This includes records like your hospital record.
