Get help for any health matter

Please do not use this form to order prescriptions. If you wish to order your prescription, please use Patient Access.

Use this form at any time to get help from the practice, for example to request a consultation with a doctor. We aim to respond as soon as we can and within 4 working hours. This may mean that we get back to you on the next working day if you contact us late in the afternoon.

Please note that our opening hours are 08:00-18:30, Monday to Friday. We will not usually deal with requests outside of these hours unless you have been advised differently.

Please note that if you contact us after 17:30 we may not be able to deal with your query until the next day. If it is very urgent, then please call the practice instead on 0161 881 4545.

All queries will be answered by a member of the Chorlton Family Practice Clinical Triage Team.

You can be assured that the advice you receive will always be written by one of the Doctors, or Advanced Nurse Practitioners, and will be appropriate to your query.
