Get Help for a Medical Problem

All queries submitted using this form will be reviewed by a doctor or Advanced Nurse Practitioner (ANP) and either responded to by the Triage Team on the day, or one of our clinical administrative staff who will advise you as to how your request will be handled.

Please note that using this form does not guarantee that your request will be dealt with more quickly, only that it will provide the clinical team with information to give you the best advice about your problem.

If, after reviewing your message, the doctor or ANP still feels that self-help or simple measures are most appropriate to deal with your request, then they will direct you back to those resources. Due to the volume of requests we receive we are unable to enter into detailed discussions about complicated problems. You will be offered an appointment to discuss your problem further if we feel it cannot be solved online.

Please do not use this form to order prescriptions. If you wish to order your prescription, please use Patient Access or visit the Register for Online Services page if you are not yet registered with Patient Access.

Please note that this form closes at 17:00 but the practice phones are open until 18:30.

Please note that if you have an acute problem needing attention and you are away from home, then you may be directed to local care services to be examined and receive treatment.

About the Matter

Please provide as much information about the problem as you can, to enable us to help you as best we can. For instance, if you have pain, where is it, what does it feel like, does it travel anywhere else? Have you had any other symptoms that you think might be relevant? Have you tried anything to help with your problem so far?

If you're not sure, it's helpful to tell us whether it's been days, weeks, months or years.
Video Consultation

Video consultations require you to have a smartphone, tablet, laptop or any computer with camera and microphone or headset. You will also require access to the internet, preferably over WiFi. If the practice decides on a video consultation you will be sent an email containing a link for the video consultation, so it's best if you can receive your emails on that same device.
