Getting the help you need from Chorlton Family Practice

MEDICAL EMERGENCIES (if it’s not a medical emergency scroll down to ‘how to access the online practice appointment system)

If you have any of the following symptoms, you should call 999 as this could be a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk. Genuine, life-threatening emergencies include [but are not limited to]:

  • Chest pain / breathing difficulties / heart attack
  • Stroke
  • Severe bleeding
  • Severe allergic reactions
  • Severe burns or scalds
  • Serious head injuries
  • Major trauma such as a road traffic accident or a fall from height

How to access the online Practice Appointment System

  • The portal is open 7-10am, Monday to Friday. It may close earlier if we are extremely busy that day.
  • If you are unable to use the online form and have additional needs please telephone the practice on 0161 8814545, if the phone lines are busy you can ask for a ‘call back’ to save you waiting in the queue.
  • You will either receive advice by email, or be phone or texted to offer you an appointment. Please keep your phone nearby and off silent, and check your email regularly for a response, including your junk folder.
  • Which GP do you want to see? The list below is a guide. If your preferred GP is not in that day, on leave, or are the duty doctor you may be allocated another GP.

  • Dr Karim Adab – Care of the elderly, particularly patients with dementia, heart failure & complex medicine. He is one of the accredited GP trainee supervisors at the practice and one of the partners with managerial responsibilities.

  • Dr Megan Atherton – Her interests include Gynaecology and women’s health. She is Safeguarding Lead for the practice

  • Dr Ves Chavdarov – His interests are Diabetes and complex medicine. He is Diabetes and Palliative Care Leads
  • Dr Chavdarov is in clinic at the practice on TUESDAY, WEDNESDAY, THURSDAY morning and FRIDAY all day
  • Prof. Carolyn Chew-Graham OBE – Is Mental health lead, and has wide expertise in unexplained symptoms. Prof Chew-Graham is practice Research lead.

  • Dr Louise Freedman – Her interests are Dermatology, women’s health. She is Deputy Safeguarding Lead and lead for Learning Disabilites.
  • Dr Freedman is in clinic at the practice on WEDNESDAY, THURSDAY and FRIDAY
  • Dr Duncan Hill – Is Minor surgery lead and interested in musculoskeletal (MSK) problems. He is Cancer Lead for the practice

  • Dr Helena Mulkeen – Her interests include complex medicine and men’s health. She is lead for GP training at the practice and one of the partners with managerial responsibilities.

  • Dr Martin Siebert – has an interest in Care of the elderly and looks after the local nursing home at Yorklea.

Community services for minor illness and urgent treatment

  • See a local pharmacist without appointment for treatment of the following 7 conditions
    • Impetigo (skin infection)
    • Sore throat
    • Ear infections
    • Shingles
    • Sinusitis
    • Uncomplicated urinary tract infections
    • Infected insect bites
  • For muscular or bone injuries, cuts, burns etc
  • Attend A&E only for medical emergencies needing urgent attention
  • Please note that doctors are unable to help with dental issues, and if you are not registered with a local dentist you will need to follow the advice above
  • Red eyes, new flashes and floaters, eye injuries, loss of vision
  • This can be for an Long Term Condition Reviews, NHS Health Check (5 yearly), cervical screening appointments and immunisations
  • The NHS App is the best way to request your ongoing prescriptions
  • If you need a medicine not listed on your app, please use the form above
  • For all Admin enquiries (eg requests for letters, updates on hospital referrals, changes to your contact details), or general non-medical enquiries, please use the form above
  • We will respond to requests within 2 days of receiving your request (between 8am and 6.30pm, Monday to Friday – excluding bank holidays)
  • For a new or ongoing problem for which you have been off work for more than 1 week
  • Please note that Fit Note requests for New problems may require a GP review
  • Please note that any request for Fit Notes for hospital procedures (eg surgery) will need to be fulfilled by your hospital clinic

PHARMACY FIRST SCHEME

Did you know you can now get help direct from your pharmacy and receive prescribed medication without the need to make a GP appointment? The scheme currently covers assessment and treatment for the seven conditions below. Your prescription will still be free if you don’t currently pay for prescriptions.

Visit your pharmacy should you have any of the above conditions, you can get the help you need, including prescribed medication if this is appropriate, quickly without the need for a GP appointment

DENTAL PROBLEMS

If you think you need urgent dental care, contact your usual dentist, if you have one. You can also contact NHS111 who can put you in touch with an urgent dental service. Please do not contact the practice as we cannot offer urgent or emergency dental care.

INJURIES

If you have an injury such as sprains, strains, suspected broken bones, injuries or cuts you will need to go to an urgent treatment centre. The closest UTC is located at:

It is recommended that you use NHS 111 before you attend, as often they can send your details to the service so they know when to expect you. You don’t need an appointment to visit an Urgent Care Centre, you also don’t need to be registered with a GP or have a fixed address.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

  • if there’s a specific doctor, nurse or other health professional you would prefer to respond
  • if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
  • if you need an interpreter
  • if you have any other access or communication needs

Home visits

If you are housebound and need a home visit, we will phone you first to understand how best we can help

To request a home visit, it’s helpful if you contact the surgery as soon after 8am as possible.
