Patient Notice

We are open Monday – Friday 8am – 6:30pm (closed on weekends and bank holidays). We take urgent clinical requests to see GPs or Advanced Nurse Practitioners between 7am and 10am via our PATCHS form.

Please be advised the majority of our appointments are on the day so please try to contact on a day you are available to attend the practice. Routine requests – such as blood tests, smears, asthma reviews, prescriptions, fit notes etc. may be submitted via our PATCHS ‘admin query’ form between 7am – 6:30pm. We will deal with your request within 1 working day.

Patient Participation Group Meeting Update – December 2025

Thank you to all patients who attended our December PPG meeting. We had an excellent turnout and a productive discussion covering digital access, winter pressures, staffing updates and patient experience.

We shared progress since the August meeting, including three successful NHS App and PATCHS sign-up events, improvements to reception processes, and ongoing work to support patients who need help using digital tools. Feedback on PATCHS continues to be mixed, and your comments are helping us refine the system and support patients more effectively.

The practice remains very busy during the winter months, but we are working hard to maintain safe and timely access. Three new administrative staff have joined the team, and we have appointed a new GP, with nurse recruitment ongoing.

We have also updated our notice boards with seasonal health advice, vaccination information and self-care guidance, and a new staff photo board will soon be on display to help patients recognise members of the team.

Thank you again to all who contributed. Your feedback continues to shape improvements within the practice. Information about the next meeting will be shared early in the new year.

We are very keen to expand our Patient Participation Group and welcome more patients to join us. If you would like to become part of the PPG, please let us know by emailing gmicb-mh.chorltonfamilypractice@nhs.net, submitting a PATCHS form, phoning the practice, or speaking to a member of the team in person. Your voice is important, and we would be delighted to have you involved.
